You can complete the survey online, over the phone or via post. Whichever way you choose, please make sure we have received your completed survey by 5pm on Friday 19 January 2018.
Online
To complete the survey online, please click here ensuring you have the Survey Code and the Access ID ready. Your Access ID is unique to you – please use only your allocated code as it ensures that you are presented with the questions most relevant to you.
You’ll need to allow around 20 minutes to complete all of the questions. Please note that, once you have started the survey, it cannot be restarted or revisited at a later date.
Phone
You can also complete the survey over the telephone. Simply call our dedicated survey line on 01636 653 060. Lines are open from 9am to 7.30 pm, Monday to Friday, until 5pm on Friday 19 January 2018. Lines will be closed from 23 December 2017 to 1 January 2018 for the Christmas holiday.
